For applicants currently in Grade 12: Scanned copy of Grade 12 – First Quarter Report Card (SF10)
For applicants who completed Grade 12 prior to AY 2025–2026: Scanned copy of SF10 (First to Fourth Quarter with General Average)

FOR TRANSFEREES

Scanned copy of Transcript of Records (TOR) or Copy of Grades from the last school attended
